Photography Release Form (download)

Please fill out the Photo Release Form and give it to your coach or captain. We need photo releases signed by all participants (and their parents if minor). In most cases photographs will be posted in online galleries, available for purchase. In other instances, TYUL would like to use photographs of league participants and events on the website or in other design materials.
